I was hitting target dummies and an Ethereum Void Reaper Rare spawned (level 80). When i killed him, this toy was looted. Not sure how or why it spawned as i was just DPS testing on BETA.

I'm thinking it is a rare that spawns when you have the Treacherous Transmitter trinket as its linked to the Twisting Nether

This is dropped by a rare that can spawn as 1 of 4 outcomes from using the Treacherous Transmitter trinket from the spider palace raid. When you move you leave behind pink energy clouds shaped like you that dissipate and your face phases in and out of 2 different places like you're phasing in and out of reality. it's super cool. Kudos bliz on such a cool toy

Something not mentioned by the other commenters is that the Faucheur du vide de l'Ethereum that drops the Voile d’infiltration only spawned after I completed the Simon Says game. (for me is was the jumping game)

He also has dialogue on spawn and on death:
Ethereum Void Reaper says: Summoned from the darkness! My thanks, but... you've served your purpose.
Ethereum Void Reaper says: We'll... keep... coming...

EDIT: I got a second Void Reaper to spawn within about 30 uses so you can spawn another after getting your toy however it seemed to drop absolutely nothing. It had different spawn dialogue though:

Ethereum Void Reaper says: From the shadows, I come!
Ethereum Void Reaper says: Always so quick to obey voices from the shadows for a morsel of power.
