The item that starts the quest won't drop unless you are doing Torture the Torturer.

Don't know if anybody noticed, but the building housing the executioner is the exact same building young Thrall is rescued from in Escape From Durnholde. Must have used a big moving truck.

Just in case there's any confusion, the quest itself is the rod, so once you start it there's no need to kill the executioner, just deliver it to its destination.
