This can be randomly fished out of a pool of wreakage in the water. The contents vary, but I've seen some of the mid-range gems come out of it.

Odd that Wowhead is saying its no longer avail, I just pulled one out of plain water near the Nesingwary camp in Stranglethorn, had a Lesser Moonstone in it.

Bloated salmon is only one of several types of bloated fish. These fish are commonly found at the new fishing spots introduced in the patch. I've made many catches at the floating wreckage, and the occassional catch at the firefin/blackmouth spots.

Bloated fish, in general, tend to have gems inside. You also have a chance to find a magical ring or necklace.

Bloated fish have only been available since the recent fishing changes, so don't let the drop rate fool you. You can find quite a few if you stick to the special fishing spots.
