This is the Necklace reward from https://www.cuckhead.com/wotlk/item=49643 along with a Tank Trinket and Spell power ring. The thing i hate about the new head quest is it doesn't give a City wide buff like it did in vanilla wow. :'(

This necklace is a reward from the quest you receive when looting the Head of Onyxia in 10 OR 25 player mode.

The necklace is, yes, ilvl 245 even from the 10 man quest. There does not appear to be any difference between the two, except that in 25 man you have a lot more people to roll against.

On topic, this is an extremely nice necklace for physical dps, especially if you're lacking hit from the transition from Ulduar to ToC 25.

This necklace replaced my Broach of the Wailing Night, as an Assassination specced rogue.

This is a really impressive hunter necklace with the hit, especially with the hunter T9 being so stingy on hit. A nice surprise from a joke boss fight.

This is not a good choice for any DK, the Spiked Battleguard one from emblems is better for any spec. i would assume similar for ret paladin, dps warriors.

Why is the vendor price only 67s? Not that I would vendor it, but seems strange to me.

Edit: As of now (patch 3.3.5), the sell price is now 8g 64s.

This in an incredibly nice neck item if you have not managed to get anything from Ulduar 25 or ToC 10/25.

I use this myself as a retribution paladin previously using https://www.cuckhead.com/wotlk/item=45819 from Emblems. Plus add to the fact that you can get this item from doing Ony 10 man as mentioned above and getting a ilvl 245 neck is extremely convenient.

collar of unending torment from 10 ToC is better than this iL 245 neck. it's too easy to be hit capped with gear now, so the 3%+ armor pen on the collar is better than the hit on this neck if you are hit capped (as any feral druid should be easily).

This is a bit disappointing for a ilvl245 neck piece when you look at it from a ret pally's perspective. https://www.cuckhead.com/wotlk/item=45819 seems to be a bit better since we gain 2.3 AP from each point of strength. However, I can surely see how someone like a rogue would die to have this (literally), what with the excellent Agi boost and all.

Got this when I won the head this week in 25 man on my DK. The hit on it is fairly useless to me-without it, I'd still be hitcapped-but as I've been trying to get more crit lately (I've got a ton of strength gear and gems), this was a good upgrade for me. Though I did chuckle when I saw the stats on it and think, "no strength, man, Blizzard must think melee is useless in this fight."
