Stormwind barber shop is west of bank opposite The Stonefire Tavern.
Look for the striped rotating barbershop pole on the ground just outside the entrance.

They're not cheap though, a full change of style, color and beard was 11g 11s 11c for me.

SW barber shop is in the northern part of the Trade District. Cost me nothing with a lvl 1 alt, 12 gold and some change at lvl 70.

Trade District, Stormwind City (Jelinek Sharpshear);
The Commons, Ironforge (Pella Brassbrush);
Cleft of Shadow, Orgrimmar (Bebri Coifcurl);
Trade Quarter, Undercity (Naznik Sureshave);
Area 52, Netherstorm (Pulik Swiftsnip);
Cosmowrench, Netherstorm (there is no barber, but a chair);
Dalaran (Kizi Copperclip).

There are two barbershops for each faction and three neutral ones.
Edit: Added the barbershop in Cosmowrench.

The barber can apparently re-attach my bottom jaw as an undead getting a hair cut.

Various hairstyles that have been made available through the barber are still not available during character creation, so imo they should still charge for the new styles even at lvl 1.

Price depends on the customers level, as Tonelimefizk said, but also changes based on work done. Changing both your hair and features will cost more than changing just one of the two.

Free Haircut:

Que up for a Battleground, walk to barber
When BG is ready to enter, sit in the Chair
Choose hairstyle of choice, do not pay
Enter Battleground, you have your hairstyle

Added: This works upon taking Deserter, and I was not charged for my pigtails <3
Added: Logging out will cause your old haircut to grow back.

You could just pay the ~15g? Would save you the hassles if you are doing that every time you log on.

A good method to 'preview' your hair cut is to use the in-game equipment manager.
For example 'DPS' and 'PvP' sets on your action bar, press DPS and you will change to your DPS gear if you have configured it properly, and click PvP to change into your PvP gear, or whatever two sets you use.

Changing gear sets refreshes the hair style on your icon (top left) and character sheet when you press C.

I found this pretty useful because I wondered how my icon would look with certain styles/colours, spending a lot of gold in the process.

Up the stairs from the Stormwind barber shop there are two skeletons buried under the floor boards next to a table with a very mean looking straight razor. Remember to tip your barber well !
