It is contained in the Unsealed Chest dropped by Arthas if at least one raid member has the quest The Lich King's Last Stand.

Bring it to Darion Mograine and you will be rewarded with the Reins of the Crimson Deathcharger, an Acherus Deathcharger looking like Iydallus, Darion Mograine's mount.

Even if The Lich King's Last Stand isn't turned in, the chest will only drop once, even if the quest is abandonned and taken back.

The soul of Alexandros Mograine, the Ashbringer. He was harvested by Frostmourne, and later risen as a Death Knight to serve the Lich King's most trusted lieutenant, Kel'Thuzad. Alexandros was one of the key founders of the Scarlet Crusade, before it's imminent corruption.
