Is the entry automatically accepted by completing the survey?
This survey will not be acurated for me as it forced me to choose something, among options don't fit my playstyle.For example, size of raid, I don't raid but I choose 4 because the question needed an answer.There are other two questions were no choice fits but I just choose one randomly.
I don't comment much on anything but I gotta say this was a really interesting survey and one I actually enjoyed filling out. I actually had genuine answers for every single question on it, as opposed to most surveys (how was our service, give us feedback on x, etc...) where as with this one I actually answered every question personally and didn't just click something cause I couldn't answer it. Great survey!

The question about PvP size should specify the kind of PvP combat. For example, I'd prefer 1v1 arenas but 5v5 matches (like LoL matches)

The youtube question was missing an option (for me at least) I rarely watch video games on youtube (I'm a car nerd so i watch motor related stuff) You should have had an other than games option there.
