I have more than 10 followers 655, 22 kills in Heroic, havent seen the Mythic quest yet. Sucks but oh well :C
The 15 number is a rough guide for it. You can definitely get the mission upgrades at less.

I got my heroic cache with less than 10 normal kills and like 3 heroics.
Here's some important clarification:As soon as you have 2 or 3 followers with 645 ilvl you will instantly get a Highmaul raid mission. The ilvl of the cache reward is determined instantly when the mission shows up -- so if you're near to getting 15 boss kills in heroic or whatever, you can delay gearing up some of your followers. You should also delay in gearing your followers this high until you have multiple epic level 100 followers, so that your chance of success in each mission is decent.You have roughly 2 days to start this mission before it disappears (try to assemble the best team possible in that time).Roughly 14 days later, another Highmaul mission will show up, and the cache reward ilvl is redetermined when this mission spawns -- so hopefully you've reached your 15 boss kill mark. You will see a new highmaul mission every 14 days after the previous -- this is the cooldown.Note that you are not restricted to seven boss kills in a given difficulty in one week! Get a new raid leader and kill more bosses (but you won't have a chance at loot).Finally the "perfect team" of seven followers (the absolute minimum that can counter all 4 missions perfectly) would be the following counter pairs (there are a few permutations, but due to the way abilities are restricted by classes, I believe this is by far the most attainable):Deadly Minions / Timed BattleWild Aggression / Danger ZonesMagic Debuff / Minion SwarmsTimed Battle / Massive StrikeDeadly Minions / Powerful SpellGroup Damage / Magic DebuffGroup Damage / Minion SwarmsGood luck!-Mang

Also I like in particular how you mention how the caches "upgrade" since it's not really too clear (it wouldn't be too far-fetched to assuming defeating Mar'tok on a difficult equates to unlocking cache rewards for the higher difficulty, which was what I assumed until I learned of the information myself).
Thanks for this guide, now I have a plan to optimize better my line-up:1. Get Leorahj and Blook, then if you can Phylarch and Nat Pagle. Upgrade them.2. Use the lunarfall inn to get a follower with Deadly Minions + Timed battle, and another with Wild Aggression + Danger Zone. Upgrade them.Then start to work with racials bonuses, which means:1. Kick all the non-ally/horde followers (arrakoas, robots, etc) except of course blook & phylarch.2. Check the followers I have for 2x bonus racials (ie: Draenei with +dwarves and +humans)3. Then use the inn to get matching racials (ie: I take Friend with argus and I pick a dwarf. Then next week I finish my team with the last race needed, the human).4. Then pray RNG gods to get some nice combos, the ultimate optimized should be 6 racials buffs (45%), ilvl 655 (5%) at least 3 counters (another 45%) and another 5% buff for a 100% team working in every mission.
They have to put some sort of mechanism to blend PvP into garrison mission system, which has become a huge part of the game now.Off the top of my head I think of a PvP epic gear version of these very caches. You can swap raid boss kill requirements with rating requirements. At the moment I feel like PvP players are largely excluded from many key aspects of the WoD.In addition, the gladiator's sanctum is sort of a failed attempt on that part. One really simple, yet cool and rewarding inclusion would be putting battleground win dailies rewarding a small amount of conquest. Honour is dime a dozen, no one needs any more of it I assume!

My experience suggests that anything less than 90% chance of success is an almost guaranteed fail, while anything above will almost always succeed (as you'd expect).I would love to see a bit of clarity from Blizzard on how the success chance is worked out, because something definitely seems shonky.
