Tempest keep is only the second hardest keyed instance to access. Doesn't require you to set foot in gruul's, karazhan, or magtheridon. Theres still loot to be seen from vashj, hyjal, and black temple.

Far too early to say anything about the casual to endgame gear-gap.

I think you might be confusing the non-raid part of Tempest Keep with the raid part. There are 3 5-man instances in Tempest Keep, and 1 25-man raid instance. The raid instance is comparable with Serpentshrine Cavern, another 25-man raid instance in Coilfang Reservoir.

To access this place, you need to have done the Cipher of Damnation questline (http://www.cuckhead.com/?quest=10588, scroll down to the comment of marskid2 for the complete questline) in Shadowmoon Valley, then do Shattered Halls (http://www.cuckhead.com/?quest=10884), Shadow Labyrinth, The Steamvaults (http://www.cuckhead.com/?quest=10885) and The Arcatraz (http://www.cuckhead.com/?quest=10886) on Heroic Mode. Then, you need to defeat Magtheridon (http://www.cuckhead.com/?quest=10888), so yes, you have to set foot in Magtheridon's Lair.

The bosses in this place can drop the Tier 5 set pieces (like http://www.cuckhead.com/?item=30247 for example, http://boards.worldofraids.com/topic-2855-1.html has another piece that does drop from a boss in here), just like in Serpentshrine Caverns. Proof from the official Blizzard website: http://www.wow-europe.com/en/info/basics/armorsets/, look all the way down to the comments to the sets:
"Tier 5 raid set: This armor can be obtained from Kelara (Aldor) or Veynna Dawnstar (Scryer) in Shattrath City using tokens obtained from Serpentshrine Cavern and Tempest Keep."

On to this particular neckpiece, it seems to be rather balanced, it's not earthshaking, but that's probably because there are just so many new and mighty items that shipped with TBC that this just doesn't seem to be too good. However, when compared with other neckpieces like http://www.cuckhead.com/?item=30099 or http://www.cuckhead.com/?item=30022, this item doesn't seem to be too bad. Also, when you use the formulas described on this page: http://www.wowwiki.com/Formulas:Item_Values, the item value you get is quite a bit higher than those 2 other items.

Conclusion: It might seem to be underpowered, but it's probably not.

well looking at the name it's probably a reward you get in for turning in Kael's head? just like Onyxia and Nefarian?
I heard Blizzard was working on giving one player in the raid Vasjh and Kael rewards if you killed them.

One of the rewards from the quest called Kael'thas and the Verdant Sphere (completed by killing Kael'thas Sunstrider, the end boss in The Eye). http://www.cuckhead.com/?quest=11007

This has more avoidance than any other necklace in the game. Most definetly worth obtaining for an avoidance set for a tank.

One of the best Prot Pally necks in the game currently. If not the best.
Roll/bid priority should go to tanks for the Sphere. Every other class can get better from their Guardian amulets from PvPing. If you argue with that, you're wrong. Just wrong.

Ive been looking around level 75-80 necks and this appears to be the best neck outside of heroics/raids. Due to the avoidence and mitigation, the best neck ive found has less of both but it does have like 15 more stamina and it has strength....So if your good on that, you would probly go for the other one which still requires level 80 so I will be using this til level 80 and possible depending on if i need the avoidence/mitigation or not ill be bringing it into heroics/raids at 80...
