Outdoors at the entry to LIVE Stratholme. Go to the right side, there is a very large tree near the water's edge (behind a large stone pillar, that's part of the facade). It's on the trunk of the tree nearest the water.

If you are on the bridge, walking toward the dungeon, follow the shore once you cross the river/get off the bridge.

Pass some flaming rubble in the water to your right, a..pulsating stump on your left, a big stone pillar on your left, and the tree you want is right here. If you followed the shore, it is the first tree you will see past the white pillar (corner of the paved road), and the eye you want to loot is facing the water on the south side of the tree.

For some reason I also already had this unlocked and was unable to loot it from outside Stratholme. I think one of the other Grimoires flags this as obtained as well.

The hidden tracking quest is 79449 so you can use this script to check if you have collected it:
/dump C_QuestLog.IsQuestFlaggedCompleted(79449)
If it returns "true" it means you have already collected it.

The eye was not there because it was already unlocked for me.

SPECULATION: I had the glyph of the observer before 10.1.5 when the new warlock pet customizations were made available.
Maybe that's why it was already unlocked for me?

Hmm - might be a small bug. For some reason this is the only one of all the grimoires that I have unlocked that is showing unlocked on the one character I unlocked on but is still showing as locked on all the others....
