If you are like me and want to farm these on all alts (for bonus and Soridormi rep) here is the macro to check what you have completed so far on the current character:

Confirmed it drops on Illidan's rift. But the particular phase/location seems random. You can get it anywhere.

Quest offered by Ellemayne during the Kill Fel-Touched Kaldorei time rift task. Only possible to complete during a Demon time rift.
