Out With The Old...
In the original versions of Diablo II, including the Lord of Destruction (LoD) expansion, there were Runewords that could only be created by characters playing that Ladder. After the Ladder season ended, these characters could bring these Ladder-only Runeword items into normal play, but they could not recreate this Runeword outside of the Ladder.
For example, the super-popular Mercenary runeword Infinity used to be a Ladder-only Runeword prior to D2:R. This meant a player could only create an Infinity polearm while on the Ladder, but could bring it back to normal play when Ladder was done. However, if they found a better polearm and wanted to recreate Infinity in normal game play, they could not do so because the Runeword would not form correctly outside the Ladder.
While this is close to how it will work in D2:R's first Ladder season, the ending will be slightly different.
...In With The New
While it was announced that D2:R's first Ladder season would introduce a few new Runewords, these will work a little differently than they did in old-school Ladder play.
When the Ladder starts, these new Runewords will only be accessible and usable by players on the Ladder, not to non-Ladder or offline players. This will be the case for the duration of the Ladder season.
However, once the Ladder season is over and Ladder characters return to normal play, those Ladder-only Runewords introduced in that season will be released to the wild -- all players will be able to use and create these formerly Ladder-only Runewords, including in single-player offline mode. In other words, new Runewords will be introduced during Ladder, exclusive to Ladder during the Ladder season, and then become generally available to all players in all game modes once the Ladder season has concluded.
This is how new Runewords are likely to be introduced to D2:R going forward.
Will There Be New Runewords Every Ladder Season?
No. While this is a planned feature for the upcoming Ladder season in early 2022, they made it clear that new Runewords would not be a feature of every Ladder season that follows. While they plan to introduce new Runewords in some Ladder seasons, there will be other perks and prizes available to keep things fresh and interesting.
What New Runewords Will Be Introduced?
No specific recipes were offered, although there was a clue that they planned to bring back versions of two old mystery Runewords that were found years ago in game files that never made an appearance in game, Plague and Pattern . They did say that they wished to make some Runewords that used lesser-used Runes, and that they wanted to fill in some gaps in the current Runewords. Once the PTR starts for this patch starts in early 2022, we expect to have more information about specific Runewords being tested.
While they made it clear that the new Plague and Pattern Runewords would be altered from the game-file versions that have been floating around fansites for years, these are the original configurations. Expect these to change, but it's interesting from a historical perspective. Another interesting fact: although these Runewords use some higher level runes (Cham is rune #32 and usually only accessible above the mid-60s), the level requirements for items created with these runewords was originally level 1! This brings in some interesting twink gear possibilities, along the lines of using the Gem of Ease in Diablo III.
Again, these are not the final form these will take if and when they are introduced.
We also do not know if versions of these Runewords will be among those introduced in the first Ladder season.

Will Runes Ever Stack?
Sadly, the answer appears to be no -- at least for any time in the near future. There were clearly no plans to allow Runes or Gems to stack, and some concern that this would break some of the original restrictions of D2. There was some vague discussion about the future possibility of a currency tab, but it was clear this kind of accommodation is not something under serious consideration for the near future.
