. This guide provides tips and strategies for defeating the Patchwerk encounter in WoW Classic Naxxramas.
Patchwerk is one of the most infamous raid bosses in WoW's history, because of its unique mechanics and excruciatingly high health pool (nearly 4 million Health, the most of any boss in Vanilla)! Not only does Patchwerk have a ton of health, but the primary mechanic in this fight is determined by how much health the tanks have, as well!
This guide provides tips and strategies for defeating the Patchwerk encounter in Naxxramas in Classic WoW.

Tanks are the most important, by far. This fight is entirely based around how well the Main Tank keeps threat over everyone else, and how well the Off Tanks can soak Hateful Strikes from Patchwerk. Essentially, the entire melee group needs to stay under the Main Tank and the 3 Off Tanks on threat, while the Off Tanks stay healed to full health throughout the entire encounter.
Patchwerk Quick Tips
Tanks
The Main Tank on the fight has to prioritize being top aggro, above all else. The only incoming damage on the Main Tank will be auto attacks from Patchwerk, so the damage won't be insane. Patchwerk should be tanked near the slime river, allowing melee to step into the slime to lower their health pool, if they need to.
Off Tanks have to prioritize having as large a health pool as possible, while keeping up enough mitigation to handle incoming Hateful Strikes. On top of that, they need to stay above all other melee on threat, as Hateful Strike can only target the top three on threat within melee range, after the highest aggro. So, the raid needs 3 Off Tanks for this encounter. Because of the insanely high health pool required, items like

Melee DPS
Melee need to stay under the four tanks on this fight, to avoid Hateful Strike targeting them. If a

damage dealer (or a
潜行者
without Vanish up) ends up getting too much threat, they need to step into the slime and immediately lower their health total, to avoid becoming a target of Hateful Strike.
Ranged DPS
Ranged can pump as much damage as they can on this fight, as long as they don't pass the Main Tank on threat. They can pass the Off Tanks, as Hateful Strike only targets players within melee range.
Healers
Healers need to focus heals on Off Tanks, keeping them at full health at all times, and keeping the Main Tank up as well, but they will have less burst incoming damage, as it will only be auto attacks hitting the Main Tank. Healers should also be careful and not be randomly raid healing, as it could lead to a scenario where a melee intentionally lowered their health in the slime because of threat, and your heal could put them as a target for Hateful Strike!
Before Pulling Patchwerk
The tanks should get plenty of time to build as much threat as possible, before melee move in and start doing damage. If the tanks get enough threat to stay ahead, melee won't have to stress as much throughout the fight, potentially leading to an aggro situation and creating a wipe. Understanding how Hateful Strike works is all the raid needs to do, as this fight is relatively straight forward, with only one primary mechanic.
Patchwerk Abilities and Strategy
As previously mentioned, Hateful Strike is really the only mechanic in this fight, so the entire raid really needs to understand how this ability works, how Patchwerk targets people with it, as well as how to utilize the environment's mechanics to ensure the raid can control it.

is a very straight forward ability that does an absolute monster amount of damage (typically around 8,000 damage), with three primary mechanics:
Hateful Strike will only target players within melee range.
Hateful Strike will only target players between second, third, and fourth on threat.
Hateful Strike will only target the player with the most health.
Understanding these three mechanics is key. It will never hit the person with the most threat, so that means the Main Tank is only taking damage from auto attacks. It goes off roughly every 1.5 seconds to 2 seconds, meaning healers need to top off the Off Tanks at all times. Because Hateful Strike is only melee range, ranged damage dealers are free to pump as much threat as they want, as long as they don't pass the Main Tank. Because of all of these factors, if a Melee were to creep up into positions two, three, or four on threat, they can intentionally step in the slime river and lower their health pool, preventing them from becoming a target to Hateful Strike (because their health will be lower than one of the off tanks).

happens at 5% of Patchwerk's total health, increasing his attack speed and damage dealt. Most Main Tanks shouldn't have a problem with it, as Hateful Strike will still only target the off tanks. But if a Main Tank is fearful of this incoming damage, they can also pop

for the Enrage phase.

is actually a spell that was added in a later patch in Vanilla, because some clever guilds found a unique kiting exploit for this boss encounter, but it took a really long time. After 7 minutes, Patchwerk will hard Enrage, increasing his damage output to insane numbers, where even players in Wrath of the Lich King gear at Level 80 would get one shot!

is similar to the Berserk ability, where after 7 minutes, Patchwerk will begin machine gun firing Nature damage Volleys at the entire raid, pretty much instantly killing everyone within range.

Patchwerk did not have the most health of any raid boss in Vanilla. Both Thaddius and Loatheb had more health, though this was compensated by DPS-boosting mechanics in both cases.

Because Hateful Strike is only melee range, ranged damage dealers are free to pump as much threat as they want, as long as they don't pass the Main Tank.
This note might have to be revisited. From the experiences in our guild, if a ranged-dps pushed beyond slot 5 (into aforementioned slots 2,3, or 4), Patchwerk would just have less targets to choose from.
For example, if a warlock hopped up into threat-slot 3, only tanks in threat slots 2 and 4 would be receiving hateful strikes, and they would receive them *much* faster.
