It is contained in the Unsealed Chest dropped by Arthas if at least one raid member has the quest The Lich King's Last Stand.

Bring it to Darion Mograine and you will be rewarded with the Reins of the Crimson Deathcharger, an Acherus Deathcharger looking like Iydallus, Darion Mograine's mount.

Even if The Lich King's Last Stand isn't turned in, the chest will only drop once, even if the quest is abandonned and taken back.

The soul of Alexandros Mograine, the Ashbringer. He was harvested by Frostmourne, and later risen as a Death Knight to serve the Lich King's most trusted lieutenant, Kel'Thuzad. Alexandros was one of the key founders of the Scarlet Crusade, before it's imminent corruption. Alexandros Mograine, known as "The Ashbringer", was one of the greatest paladin there have ever been. Sadly he was killed, by he's own son.

Alexandros was the greatest Paladin ever. He (along with his blade) were the Ashbringer, the sword and the Highlord together. Death feared and trembled before him. He would destroy Arthas had he not been betrayed by his son Renault.
