This item was dropped by Balzaphon during the Scourge Invasion event after the 1.11 patch. I don't think there's any way to get it now.

This neck dropped off a special boss in Strat as part of the Scourge Invasion around Naxx release. As that is over, the boss is no longer there, so there is no current way of getting this neck at the moment legally.

Dropped by Balzaphon, whose other drops (besides two Corruptor's Scourgestones) are:
Staff of Balzaphon
Waistband of Balzaphon

To reach him from the service entrance (backdoor) to Stratholme, enter the first room then take the passageway west, on your left. From that room, take the road downhill to your right (north) and he'll be backed against the wall where the road curves west again.

If you do not have a Key to the City, go in the front entrance normally and proceed to the first intersection; take the road left, then at the next turn right through the AoE trap, then right again after that. He'll be against the wall where the road turns south. (You may, if still keyless, consider at this point going onwards towards the service entrance, using the first route listed here in reverse-- Magistrate Barthilas will be standing in that room, and will drop the key if you kill him.)

Dropped off a lich looking like Araj the Summoner but inside Stratholme. We did a baron run and I ran back thru to entrance to Baron and there was a lich wandering around by the gate to SC side right near Hearthsinger. His name was Balphazor or something of that nature. Very easy boss to take down, did it with just me and a lock. at 30% he casts a fear and frostbolts like crazy but has no armor and easy to take out.

i did 24 runs with my mage leading, my friend priest went and got it 1st run, then we tried it with him, putting him to party leader, and BAM it came. 24=0 necks with mage but with priests like 100% drop change :P

its all about the partyleader uniq id.

got it off the new mob in DK with a 3men team - mage,lock,priest.

go inside DK, at the first junction, u'll see grps of skels just turn to left after u killed the first grp of skels. u'll see something new floating around grps of ghouls and skels there.
