A guide for Horde to complete both Extinguishing the Cataclysm and Flame Keeper of Cataclysm:

Portal to Uldum
at (53.2, 34.4)
at (53.4, 32)
Hearth back to Orgrimmar, Portal to Twilight Highlands
Fly to Bloodgulch for at (53, 46.2)
Fly to Thundermar for at (47, 28.2)
Portal back to Orgrimmar, Portal to Deepholm
at (49.4, 51.4)
Portal back to Orgrimmar, Portal to Mount Hyjal
at (62.8, 22.6)
Portal to Orgrimmar, Portal to Vash'jir
Depending on your phasing, you might have to swim to Silver Tide Hollow
at (49.3,42)
Assuming a 15 min timer, your hearth should be up to return to Orgrimmar

right to your back if you look at the ally fire you have the

horde one in the back just a few steps away from the ally one

If you can't see the one in Twilight Highlands you'll need to do some more quests there.

You can use the TomTom addon and put these lines in a simple macro that will show you all the points with (H) for Horde and (A) for Alliance.

/way Twilight Highlands 53 46 (H)
/way Uldum 52 34 (H)
/way Uldum 53 31 (A)
/way Twilight Highlands 47 28 (A)
/way Mount Hyjal 62 22 (A)
/way Deepholm 49 51 (A)

/way Vashj'ir 49 41 (A)
These last coords are in the Shimmering Expanse so, use them once you are there.
