For this Achievement (and Boss) you need really big DPS.

The Boss will cast The Skullcracker spell on 66% and 33%. He is also casting Quake.
This spell will summon 5 elementals with really big dps. You need to have boss on 66% hp when he will cast it. When he will pull you (Chains of Woe) and use The Skullcracker, he will kill all elementals around him (and you if you dont leave chains). You need do this twice to earn this achievement.

This Achievement is not easy to do but if you kill the boss, you will probably earn it because its easiest way to kill him.

make your tank gather all the elementals around the boss and use defensive cds to survive the skullcracker. Shield wall on first time and Last Stand on second should be enough to survive.

In Heroic Mode, the boss will cast a quake that spawns an Angry Earth Elemental underneath each of your group members. He then chains the entire group next to him, and you must quickly kill the chains before you can move out of range. At the end of his channel, he will then use his Skullcracker ability to kill the entire set of Angry Earth Elementals as long as they remain close by.

The achievement bugged out on me twice. The first time we waited 2 quakes and let him kill them, 10 elementals, killed him but no achievement. Today, we let him cast 3 quakes, 2 ran out but he killed 13 elementals, no achievement.

Since the adds are elementals, Paladin tanks can also cast a glyphed Holy Wrath after destroying Chains of Woe to stun the adds in place and run away.

If I understood this right, the Angered Elementals have to survive from 66% to 33% of boss' health when he'll cast his Skullcracker again? Or can I do it at once?

You just need to not dps the adds and snare them after you kill the chain. A mage's frost nova or paladin's glyphed holy wrath will do the trick.

You will not be awarded the Achievement until the boss is dead, which makes tracking a little difficult.

For anyone crazy out there such as myself thinking about soloing this achievement, don't bother. Quake only spawns 1 Angered Earth per player and it seems they die automatically after about a minute resulting in you having a maximum of three up for any Chains of Woe.

In short: 3 adds max times 2 Chains of Woe in the encounter = 6 adds killed max, 4 short of the achievement.
